This new position is an opportunity to lead an engaged team of professionals to develop and integrated effective model of community case management and care coordination with patient at the center.  As the Executive Director you will be positioned for a high degree of influence with executive team.

Offering relocation expenses, a 15% bonus structure, and a base salary starting at $102,000, this health care organization is one of the premier places to work in South Bend.  The position is open to all qualified candidates in the US.  At this point no remote work or sponsorship is offered for this position.

Team Structure: 3 Direct reports initially, 77 FTEs indirectly with relationship lead responsibility for Physician Advisor and the Post Acute Aligned Skilled Facility network group of administrators.

Reports to the Chief Financial Officer. Responsible for the development, implementation and ongoing review of the enterprise wide continuum of care case management integrated system. Provides leadership and direction to implement the case management plan.

Scope of the position includes:

  • Hospital utilization review

  • Hospital transition planning/social services functions

  • Care Coordination Institute (CCI) long term complex case management and risking risk intervention program

  • Transitional care program

  • Post-Acute Care Team (PACT) readmission reduction program

  • Palliative Care Services (inpatient and outpatient)

  • Hospital based chronic disease management services

  • Post hospital discharge call back/follow up programs

  • Diabetes Education

  • Aligned post-acute partner relationships

  • Physician liaison

Other case management programs to be determined based on need

The organization is considering candidates with the following experience:

-Minimum 5 years of case management experience & 3 years management experience required
-Knowledge of population health strategies, ACO and risk sharing approaches
-Demonstrated success with working horizontally and vertically with progressive advancement
-Proven strategic thinker that has successfully executed and sustained results both economic and quality related
-Experience in hospital and post-acute care Case Management and Utilization Review, multiple years
-Master’s Degree or other advanced degree in Nursing (preferred)
